In Terry Gilliam's "Brazil" the inciting incident is a fly getting caught in a typewriter as a drone types an arrest order for a political dissident, leading the police to raid the wrong house and kill an innocent man. Crazy shit.

The investigator also wildly fucked up. The surveillance video shows someone decades younger. Not just AI caused this travesty.
